Review medication errors
What raises a medication error, how a manager or deputy closes one, and who can close the ones you cannot.
Closing is decided by who recorded the event, not only by role: the person who recorded it cannot close it, and the company's Responsible Individual and company admins can close errors a home's own manager or deputy cannot.
ARC notices certain things in the medication record and puts each one in a queue for a manager to look at. Some have a simple explanation. Closing one records what you found and who you told.
When errors are raised
Only once your home is live on ARC. While the home runs alongside the paper MAR, ARC raises no medication errors, and after going live it does not raise errors for doses due before that moment.
The types you will see are:
- Missed dose — a regular dose with nothing recorded by the end of its window. ARC checks every five minutes and tells your managers and the Responsible Individual.
- Dose not given, and Late entry, from the round.
- Dose unlocked — every approved unlock.
- Recorded despite a check — a dose given despite a safety check: over a when-required limit, by someone without in-date training, or from an unchecked prescription.
- Count does not match — a stock count that differs from ARC's balance.
- Entry amended — every amendment.
If a missed dose is later recorded as a late entry, ARC sets the open missed-dose error aside, links it to the late entry, and lists it under Recorded late, so there is one error to close.
Close an error
- Open Medication, then Errors to review. The tab shows how many are open. Use the pills to filter by type.
- Open an error under To look at. Anything staff have flagged sits above it; see the last section.
- Say what happened and what was done about it.
- Answer who you told. For GP, Social worker and Parent or carer, choose Yes or No. Names are optional. ARC sends nothing outside the home — this records what you did yourself.
- Select Close this error. It moves to Closed in the last 30 days.
Raise an incident
Choose Raise an incident from this if the error needs one. ARC never raises an incident for you; some mistakes have a clear explanation and do not need one. You write a description, and the incident is created with that description and linked to the error. An error can have only one incident. Like any new incident, it emails and notifies every manager of the home.
When you cannot close it
You cannot close an error from your own event — for example a dose you recorded or an unlock you approved. Any other manager or deputy can, and so can the Responsible Individual or a company admin. They close errors from Medication errors to close, which lists open errors across their homes. A Responsible Individual who is not a manager or deputy of the home gets the notification link to that page.
Entries flagged by staff
Anyone can flag a recorded entry with Flag a mistake. Flags wait at the top of Errors to review. Open the young person's medication record, then Amend the entry or Dismiss the flag with a note saying why it stands.
