Care & safeguarding Children [young person] Daily life & records Activities

An activity is a planned outing, club, sport or piece of structured leisure — recorded before it happens, then written up afterwards with how it went and what the young person made of it.

Record one

Open the young person's record, then Activities under Daily life & records, and choose Record activity.

Activity title and Date and time are required. Type — Outing, Club / group, Sport, Leisure / recreation, Education / learning, Other — is optional, as are Location and Description.

Consent required for this activity is a tickbox on the form. Risk assessment note is free text for the considerations you want on the record.

If there's no active activities and outings consent on file for this young person, an amber banner appears on the page and again inside the form:

No active activities and outings consent is on file for this child. Ensure consent is obtained or recorded before the activity takes place.

It does not stop you saving. It is a prompt, not a block. The banner carries a Record consent link straight to the consents page, and clears once an active consent is on file.

Writing it up

The lower half of the form is headed Completion record:

  • Attended — Yes — attended, No — did not attend, or unrecorded
  • Outcome — how it went
  • Child's view — in their own words where you can

As with appointments, the status is driven by one thing: recording an outcome makes it Completed. Until then it reads Scheduled, whatever the date says.

Update reopens the form and only sends what you changed, so adding an outcome later leaves everything else alone.

Manager sign-off

A manager can sign off an activity — scheduled or completed — and it can only be done once. The second attempt is refused with "This activity has already been signed off."

A manager can also send a record that is awaiting sign-off back to a named member of staff for changes, or undo a sign-off if something was missed. See Send a record back for changes, or undo a sign-off.
