A LAC review is chaired by the Independent Reviewing Officer and belongs to the placing authority. ARC's job is to hold when it is due, what the home put into it, and what came out.

LAC reviews sit on the same page as the held statutory plans: open the young person, then Statutory care planning under Care, health & education, and look for LAC / IRO reviews.

Log a review

Select Log review and fill in:

Field Notes
Review Type Required
Review Number in Placement Optional, e.g. 1
Scheduled Date Required
Completed Date Leave blank until it has happened
IRO Name, Chair Name (if different)
Home's Contribution What the home submitted to or raised at the review
Recommendations / Actions Arising What was agreed

The modal is headed Log LAC Review. Save with Log review; on an existing review it is headed Edit LAC Review and saves with Save changes.

The four review types are:

  • First Review (within 20 working days)
  • Three-Monthly Review
  • Six-Monthly Review
  • Other / Ad-hoc Review

ARC prints the cadence on the page: "Statutory timescales: first review within 20 working days, then within 3 months, then 6-monthly."

Status is worked out from the dates

Status When
Completed A completed date is recorded
Overdue The scheduled date has passed and no completed date is set
Upcoming Everything else

You never set the status yourself. Log a review ahead of time with its scheduled date, and record the completed date once it has happened — that is the whole lifecycle.

Review paperwork

Once a review is saved, reopen it with Update and you will find Review paperwork in the modal: "The IRO's report, the minutes, the signed recommendations. PDF, image or document, up to 10MB." On a review you have not saved yet the modal says "Log the review first, then reopen it to attach the IRO's report, minutes, or recommendations."

Choose a File, say What it is — IRO report, Review minutes, Recommendations / actions, Consultation form or Other — add an optional Label (it defaults to the file name) and select Attach file. Any member of the home who can update the review can attach, and anyone who can view the review can download. The review's row then shows how many files it holds.

Only a manager sees the remove (bin) control, and removing a file asks you to confirm. ARC's own comment says removed paperwork is kept with the record rather than hard-deleted.

Overdue reviews

Overdue reviews are hard to miss on this page: a red banner at the top reads "n overdue LAC review(s)", names their types and says the scheduled date passed without completion, and the LAC / IRO reviews heading carries a red "n overdue" tag.

The banner is on this page only. Nothing read here puts an overdue LAC review on a dashboard.

Manager sign-off

Sign off confirms the home has reviewed and actioned the recommendations. It appears only on a completed review that has not been signed off, and only to a manager.

Two refusals to expect: "Only a completed review can be signed off." if there is no completed date, and "This review has already been signed off." on a repeat. If the review changed after the manager opened it, ARC refuses and asks them to reload: "This record has changed since you opened it. Reload to see the latest version before you sign it off or send it back."

Sending a review back, or undoing a sign-off

A manager has two more buttons on a review, shown on the row and inside the Edit LAC Review modal.

  • Send back — opens Send back to staff. The manager chooses Send to (a member of the home; the person who created the review is pre-selected) and writes What is needed?. Both are required. That person gets an email, a notification and a dashboard alert, and the alert stays until they resubmit. They edit the review and select Submit for review again. While it is with them it cannot be sent back again. The note is shown to the person it was sent to and to managers, not to other staff.
  • Undo sign-off — opens Undo sign-off and asks Why are you undoing the sign-off? (required). The review goes back to awaiting review, and the signed-off version is kept in its version history.

Send back is offered on a review that is not signed off; Undo sign-off on one that is. ARC refuses a send-back on a signed-off record ("This record is signed off. Undo the sign-off first, then send it back."). The person it was sent to, or a manager, can resubmit it.

Nothing signed is overwritten: each step is recorded as a version with who did it and why. Edits made after a sign-off are allowed, and managers see "Edited since it was signed off — the changes are in its version history."

Who can log

Any member of the home can log and update a review. Only sign-off is reserved for the manager.
