What you're seeing. A record you meant to keep looks gone.

Why it happens. Most care records can't be deleted. They are archived or superseded, and reviewed records keep a full version history — who changed what, when. A plan is the exception: the home's manager can delete one, but it is kept, not erased.

How to fix it. For a plan, the manager opens Deleted plans on the Plans page and selects Restore plan — see Delete and restore a plan. For anything else, ask a manager to check the version history. Corrections are made as new entries rather than by overwriting, so both the original and the correction stay visible.
