Incident types and extra fields
Add your own questions to any incident type, and create your own incident types.
Every company records incidents slightly differently. Incident types lets you add your own questions to the incident form, and create incident types of your own. Both apply to every home in your organisation.
Where to find it
Sidebar, Company Admin, then Incident types under Recording & compliance.
Add questions to an existing type
The page lists every incident type ARC offers, grouped by category. Choose Edit fields beside a type to build extra questions for it. You use the same form builder as for logs: short and long text, numbers, yes or no, single and multiple choice, files, staff members, dates and times. Mark a question as required if it must be answered before an incident is sent for review.
Staff see your questions on the incident form as soon as they choose that type.
Create your own incident type
Choose New incident type, give it a name, and pick the ARC type it is based on. Your type behaves exactly like the one it is based on. If it is based on Missing from care, for example, it opens the missing-episode details and the Return Home Interview, is treated as safeguarding, and prompts the manager about Regulation 40. You can't make a type less regulated than the one it is based on.
Your type appears in the incident type list under the same category as its base type. You can then add questions to it in the same way.
You can't change the base type once an incident has been recorded with your type. If you no longer want a type, deactivate it: it disappears from the list for staff, and existing incidents keep their name. A type that has never been used can be deleted.
What happens when you change the questions
Changing the questions never changes incidents already recorded. Each incident keeps the questions it was created with, so an old report always reads the way it was written. New incidents get your latest questions.
Required questions
A required question must be answered before an incident is sent for review or sent back again. A draft can be saved with it empty.
Where the answers appear
On the incident page, in the PDF, in the incident email sent to managers, in the version history, and in a child's subject access request export.
