Nobody is being asked to acknowledge the risk assessment

What you're seeing. You created a risk assessment but no one has been asked to read it, and it isn't showing as active.

Why it happens. New assessments start as pending review. A manager has to approve one before it becomes active — approval is what activates it. Until then there is nothing to acknowledge, which is deliberate: staff should not be acknowledging an assessment nobody has checked.

How to fix it. Ask a manager to review it. Approval activates it and the acknowledgement requests follow.
