My colleague can't see the concern I raised

Who can fix this: You Incidents & safeguarding

What you're seeing. You raised a safeguarding concern; a colleague on the next shift can't find it.

Why it happens. By design. Staff see only the concerns they raised themselves. The home's DSL sees all of them; a manager who is not a DSL sees counts only, not the concerns. This limits how far sensitive information travels, and it is working correctly.

How to fix it. Nothing to fix. The concern has reached the people who act on it. If your colleague needs to know something operationally, that belongs in the handover, not in the concern.

Who can fix it. Nobody — this is intended behaviour.
